At T Temple Family Health NP PLLC, we provide evidence-based Medication-Assisted Treatment (MAT) for individuals struggling with opioid dependence. Our program combines FDA-approved medications such as buprenorphine (Suboxone) with ongoing medical supervision and supportive care — all delivered through secure telehealth visits.
Our goal is to provide compassionate, confidential, and effective treatment that helps patients regain stability and improve overall health from the comfort and privacy of home.
Same-day/week telehealth appointments available. To schedule an appointment, please contact our office or book online. Dear clients, please note that drug testing would be requested and required prior to the appointment.

MAT Program Fees & Payment Policy:

Medication-Assisted Treatment (MAT) services at T Temple Family Health NP PLLC are
currently offered on a self-pay (out-of-pocket) basis only.

• We do not bill insurance for MAT visits at this time.
• Visit Fees
• Initial MAT Evaluation: $200.00
• Follow-Up MAT Visit: $150.00
• Payment is required at the time of service.

Patients enrolled in our MAT program receive:

• Comprehensive initial evaluation via telehealth
• Personalized treatment plan based on medical history
• Medication management with buprenorphine-based therapy when appropriate
• Regular follow-up visits for monitoring and support
• Mental health support when needed
• Safe, confidential, judgment-free care

How the MAT Program Works

• Schedule a telehealth appointment
• Complete medical evaluation
• Provider determines if MAT is appropriate
• Medication prescribed when clinically indicated
• Ongoing follow-up visits required for continuation of treatment
• All treatment plans are individualized based on patient needs and medical guidelines.
